What we encountered at the oncology department in Gävle was not good care, but a systematic failure characterized by a lack of competence, carelessness and a total lack of responsibility.nnThe care chain was marked by delays, missed examinations and decisions that were made without listening to the patient or relatives. Crucial questions were dismissed, symptoms were not taken seriously and when we asked for answers about the disease and treatment we were instead referred to a counselor – even though our questions were about medical errors and an obviously deficient medical record.nnThe management of medical records is a serious problem in itself. The documentation that has been provided is incomplete, contradictory and missing important parts. Notes seem to be missing, decisions are poorly or not documented at all and this raises strong concerns about what actually happened – and when.nnTreatments were changed without clear explanations, heavy medication was given despite obvious risks and when the consequences became serious we were met with indifference. Instead of responsibility, we were told that we could “seek care elsewhere” if we were not satisfied.nnThe most frightening thing is not the individual mistakes, but the whole: ignorance, carelessness, lack of empathy and a culture where no one seems to take responsibility when something goes wrong. Patient safety has clearly failed on several levels.nnThere has been a whole chain of shortcomings. It is difficult to understand how this can happen without consequences, when people’s lives are at stake.nnThis is not care worthy of the name.
